Patient privacy is a priority at Nashoba Valley Medical Center.
Release of Protected Health Information (PHI) requires patient authorization and must be signed by the patient, or the patient's legal representative (guardian or person with durable power of attorney). If a child is an emancipated minor or age 18 years or older, parents cannot get copies of their child’s medical record.

If you have a medical emergency you may contact the appropriate correspondence area at:
- 978-784-9266 during normal business hours, Monday through Friday between 8 a.m. and 4:30 p.m.
- 978-784-9000 after hours and ask for the nursing supervisor on call.
Please note: If you are requesting copies for purposes of continued care, please state this in your request. An abstract will be provided to the physician at no charge. Otherwise, a fee may be associated with your request.

Unfortunately, medical records does not provide radiology films or copies of hospital bills. Such requests should be directed to the appropriate department/site:

- Copies of birth certificates or death certificates are not available from the hospital. To obtain these, please contact the appropriate City Hall or the Massachusetts Department of Vital Statistics.
